Multiple Myeloma
Program Overview
Multiple myeloma will account for approximately 15,270 new cancer cases in 2004 and result in
an estimated 11,070 deaths. Although multiple myeloma is sensitive to both chemotherapy and
radiation therapy, it remains incurable at present and durable complete responses are rare. The
median survival with standard treatment is approximately 33 months, highlighting the importance
of developing novel treatment approaches for improving outcomes in patients with multiple
myeloma.
Patients with multiple myeloma are generally classified into four categories of prognostic
significance: smoldering myeloma, Durie-Salmon stage I myeloma, plasmacytoma, or more
advanced stages of systemic myeloma. Management of patients varies by stage of disease and
individual patient characteristics; however, concurrent with the management of specific complications,
chemotherapy should be instituted promptly to reduce the number of malignant plasma cells.
Patients with smoldering (indolent) or stage I myeloma should be observed and no initial treatment
is required; however, if their disease progresses to stage II or higher, primary therapy with
conventional chemotherapy and bisphosphonates should be initiated. De novo patients who are
eligible, should be considered for an autologous transplant as randomized trials have shown that
this approach is superior to conventional chemotherapy. Conventional chemotherapy for patients
with advanced multiple myeloma include oral melphalan/dexamethasone (MD) or vincristine/
doxorubicin/dexamethasone (VAD). Thalidomide/dexamethasone (THAL/DEX) is also an option
based on results from several clinical trials. Additionally, recent clinical findings also indicate that
liposomal doxorubicin/vincristine/dexamethasone (DVD) is also a treatment option that provides
similar efficacy to VAD while allowing easier administration and a more favorable toxicity profile.
For patients with progressive disease who received MD as their primary therapy, VAD or DVD are
acceptable treatment options. Additional strategies for salvage therapy include cyclophosphamide-VAD
(C-VAD), etoposide/dexamethasone/cytarabine/cisplatin (EDAP), high-dose cyclophosphamide,
and thalidomide. For patients who have progressed and received at least two prior therapies,
bortezomib should be considered
Ongoing clinical trials continue to evaluate novel treatment strategies in an effort to improve
outcomes in patients with multiple myeloma. Encouraging preliminary results have been reported
for one such strategy, the combination of liposomal doxorubicin/vincristine/reduced-dose
dexamethasone (DVd) and thalidomide, in patients with newly diagnosed myeloma. The treatment
of multiple myeloma is complex and a continuous reexamination of treatment approaches is
necessary to improve outcomes.
