Drug Reimbursement Information and Assistance:
05.20.09     Amgen Oncology Assistance
Amgen’s patient assistance programs are designed to provide access through free goods and other support services to qualifying uninsured and under-insured patients. In addition, Amgen makes donations to third-party co-pay assistance foundations. Amgen's programs include the SAFETY NET® Foundation, which allows access to their products for uninsured patients with household annual AGIs up to $75,000.
09.01.07     Celgene's Patient Support Coordinator Program
  Celgene's Patient Support Coordinators assist patients and healthcare providers to navigate the challenges of reimbursement, provide information about co-pay assistance, and answer general questions about Revlimid and thalidomide. PSCs can also assist uninsured and underinsured patients with enrollment for Celgene's PAP or free goods.
02.21.06     Millennium's VELCADE Reimbursement Assistance Program
The VELCADE Reimbursement Assistance Program can assist in determining patient coverage, understanding reimbursement policies as well as resolving any issuesthat may arise while requesting reimbursement for VELCADE®. Reimbursement Counselors are available from 9 am to 7 pm Eastern Standard Time (EST).
12.01.08     CancerCare Co-Payment Assistance Foundation
A not-for-profit organization established in 2007 to address the needs of individuals who cannot afford their insurance co-payments to cover the cost of medications for treating cancer.
05.29.08     Cephalon's Trisenox patient assistance/reimbursement assistance program
Patient assistance is available for certain individuals who are uninsured or who have significantly limited health insurance coverage. Reimbursement assistance is available for patients who have health insurance and need help securing appropriate coverage.
04.11.08     Chronic Disease Fund
The Chronic Disease Fund is a non-profit organization founded in 2003. They provide co-payment assistance for pharmaceutical products for patients with private insurance or Medicare part D. Patients who utilize a participating pharmacy can have their out of pocket expenses remitted by the fund directly to the pharmacy. Patients using non-participating pharmacies can submit receipts for reimbursement.
06.02.01     Leukemia and Lymphoma Co-Pay Assistance Program
The Leukemia & Lymphoma Society provides supplementary financial assistance to patients in significant financial need up to a maximum of $5000.
04.16.08     Medical Billing Advocates
They review your medical bills and insurance documents for errors and assist you in better managing your health care dollars. On average, they save between 15-30% on most health care claims and can review claims up to two years old.
02.21.06     Novartis' Zometa Assistance Patient Program (ZAP)
Reimbursement support will include clarifying insurance policy benefits, provide billing and coding information, and help identify alternative sources of coverage. Low income patients who do not have insurance coverage may be eligible for free drugs.
04.16.03     Ortho Biotech Procrit Reimbursement Line
PROCRITline provides easy access to reimbursement information and support:
*Verifies insurance coverage
*Shares insurance guidelines for private, federal, and state programs and plans
*Provides general information and assistance with product specific claims and appeals
02.21.06     Ortho Biotech's DOXILine.com
Ortho Biotech will provide assistance to certain eligible DOXIL patients at no charge, based on medical and financial need, including:
*Benefit Verification
*Prior Authorization research
*Appeal process and procedure research
*Alternate sources of payment
05.29.08     Partnership for Prescription Assistance (PPA)
Partnership for Prescription Assistance offers a single point of access to more than 475 public and private patient assistance programs, including more than 150 programs offered by pharmaceutical companies. Includes assistance for the uninsured.
04.11.08     Patient Access Network Foundation
The Patient Access Network Foundation provides financial support for out-of-pocket costs associated with a wide range of drugs, to treat a number of conditions.
05.29.08     Patient Advocate Foundation's Co-Pay Relief Program
Provides direct co-payment assistance for pharmaceutical products to insured patients (including Medicare Part D beneficiaries) who financially and medically qualify.
Housing and Travel Assistance:
04.11.08     Air Charity Network™
Formerly Angel Flight America, ACN provides access for people in need seeking free air transportation to specialized health care facilities or distant destinations due to family, community or national crisis.
02.06.02     Air Lifeline Midwest - free air travel for patients
Their mission is to provide people in medical and financial distress with access to free air transportation on small (4-6 seat), private aircraft for distant health care and other compelling human needs.
06.02.01     Corporate Angels Network - Free Air Travel For Cancer Patients
Free air transportation for cancer patients traveling to treatment using the empty seats on corporate jets.
02.06.06     Joe's House
A housing resource for cancer patients and their families who must travel away from home for medical treatment.
07.31.01     National Patient Travel Center
They facilitate patient access to appropriate charitable medical air transportation resources in the United States.
Legal Assistance:
04.11.08     Patient Advocate Foundation
Patient Advocate Foundation is a national non-profit organization that seeks to safeguard patients through effective mediation assuring access to care, maintenance of employment and preservation of their financial stability relative to their diagnosis of life threatening or debilitating diseases.
08.13.08     BMT InfoNet Insurance Help Form
If you are covered by a US insurance plan and they have refused to pay for all or part of your bone marrow transpant, BMT InfoNet can refer you to not-for-profit organizations and attorneys who may be able to help you. The referral service is free of charge. The initial consultation with most attorneys is also free. Fees thereafter vary and should be discussed directly with the attorney.

CancerCare is a national nonprofit organization that provides free, professional support services for anyone affected by cancer. Their programs – including counseling, education, financial assistance and practical help – are provided by trained oncology social workers and are completely free of charge.
