CephalonCares Foundation
The CephalonCares Foundation Patient Assistance Programs provide Cephalon, Inc. medications at no cost to patients in the United States who meet certain insurance and income criteria.

Amgen Assist®
Amgen provides a range of patient assistance programs to meet the unique needs of patients to help cover the costs of their medicines.

Celgene Patient Support®
Celgene Patient Support® is a free service that helps patients and healthcare professionals navigate the challenges of reimbursement, providing information about co-pay assistance, and answering questions about obtaining Celgene products.

Amgen's Safety Net Foundation
The Safety Net Foundation is a nonprofit patient assistance program that helps qualifying uninsured patients access Amgen medicines at no cost.

Millennium's VELCADE Reimbursement Assistance Program
The VELCADE Reimbursement Assistance Program can assist in determining patient coverage, understanding reimbursement policies as well as resolving any issuesthat may arise while requesting reimbursement for VELCADE®. Reimbursement Counselors are available from 9 am to 7 pm Eastern Standard Time (EST).

CancerCare Co-Payment Assistance Foundation
A not-for-profit organization established in 2007 to address the needs of individuals who cannot afford their insurance co-payments to cover the cost of medications for treating cancer.

Chronic Disease Fund
The Chronic Disease Fund is a non-profit organization founded in 2003. They provide co-payment assistance for pharmaceutical products for patients with private insurance or Medicare part D. Patients who utilize a participating pharmacy can have their out of pocket expenses remitted by the fund directly to the pharmacy. Patients using non-participating pharmacies can submit receipts for reimbursement.

Leukemia and Lymphoma Co-Pay Assistance Program
The Leukemia & Lymphoma Society provides supplementary financial assistance to patients in significant financial need up to a maximum of $5000.

Novartis' Zometa Assistance Patient Program (ZAP)
Reimbursement support will include clarifying insurance policy benefits, provide billing and coding information, and help identify alternative sources of coverage. Low income patients who do not have insurance coverage may be eligible for free drugs.

Partnership for Prescription Assistance (PPA)
Partnership for Prescription Assistance offers a single point of access to more than 475 public and private patient assistance programs, including more than 150 programs offered by pharmaceutical companies. Includes assistance for the uninsured.

Patient Access Network Foundation
The Patient Access Network Foundation provides financial support for out-of-pocket costs associated with a wide range of drugs, to treat a number of conditions.

Patient Advocate Foundation's Co-Pay Relief Program
Provides direct co-payment assistance for pharmaceutical products to insured patients (including Medicare Part D beneficiaries) who financially and medically qualify.
